Recently, the Iowa Department of Health and Human Services (Iowa HHS) was informed of a new substance called
Bis (2,2,6,6-tetramethyl-4-piperidyl) sebacate (BTMPS) that is being mixed with fentanyl. This substance has been
detected in the state. It’s unclear exactly what this substance does, but the linked article below suggests illegal drug
manufacturers might be adding it to fentanyl precursors or to the final product “at a high level in the supply chain,”
possibly to stabilize them from degrading from light or heat exposure as illicit drugs are made, stored and transported.

Iowa HHS encourages you to:
1. Report overdoses with atypical features to the Iowa Poison Control Center (1-800-222-1222).
2. Provide education to patients, families and community partners that:
a. Naloxone should be used for all suspected drug overdoses, including those in which fentanyl co-occur
with other substances.
b. Respiratory support (e.g., rescue breaths, supplemental oxygen) should be provided as available.
c. 911 should always be called first.
3. Counsel patients on the volatility of the illicit drug supply.
4. Make naloxone available to anyone at risk of experiencing, or encountering someone experiencing, an
overdose.
